Since 2004, Diamond Willow has provided memory care and assisted living under one roof, in small-community campuses located across Northern Minnesota.
We operate 8 communities throughout the state, including Cloquet, Detroit Lakes, Duluth (Lester Park), Grand Rapids, Little Falls, Mountain Iron, Park Rapids, and Proctor.
Each community features all-inclusive rent (no extra care service fees!) so you and your family know the cost transparently. Our person-centered care is managed by on-site registered nurses and familiar care specialists who deliver compassionate, local assistance to each and every resident.
